Although it is hard to believe, the end of this academic year is less than 6 weeks away. As in previous years we will be distributing an end of year pack to parents towards the end of this term which will include:
- School Record Data Sheet. We would ask that you carefully check the information, make any changes necessary, sign at the bottom of the sheet and return to the school office. If there are no changes please just write ‘no changes’ on the form and return it.
- Medical Form. We would also ask that parents with children who suffer from asthma visit the school office to review and sign off their child’s ‘Asthma Care Plan’ and consent for use of the school’s emergency asthma inhaler.
- Breakfast Club letter; and
- CoolMilk letter (a reminder of how to order milk for children aged over 5 should they want it).
- Term Dates.
- Privacy Notice.Â
This year we will use the Local Education Visit and Swimming consent forms that we have on file. If you wish to withdraw consent for either of these please inform the school office in writing.
You will appreciate that it is important that are records are complete and current, please therefore return the forms with your child on the first day of the term in September – Wednesday 4th September 2019 – at the latest.
